Application Forms

Hi Gang,

Here are the forms to fill out.  Go green, fill out the forms on your computer and submit them to us digitally.  All you need is the free Adobe reader available at www.adobe.com

We need the following:

  1. Candidate Profile (Complete or update annually via printed copy or electronic PDF)
  2. Pastoral Recommendation (Ask you pastor to resubmit annually via printed copy or electronic PDF)
  3. Mission Specific Application (Word Document to download, complete and submit)
  4. A recent “headshot” photo (digital or mail a copy)
  5. $20 Non-refundable Appication Fee payable to JCCY.

*Important- Download & Install the latest version of Adobe Reader (it’s free at www.adobe.com)
Complete the forms on your computer.
When you are finished, click on the submit button in the purple toolbar at the top of the page.
You will get immediate confirmation that your application is queued for submission and within a minute submission will be successful.
